We receive two types of information: (1) Information voluntarily provided by you (such as your mobile number, email and zip code); and (2) Information that may be automatically obtained through your method of accessing our site.
- Information you provide. If you choose to use our Ascend Notify™ service you voluntarily provide us with your phone number and zip code. Your phone number is stored on our system and then shared with the applicable Health Department and/or the Mental Health Recovery Board associated with your zip code. Ascend Notify™ uses the phone number to send real time SMS text notifications related to the alert topics that the Health Department and/or the Mental Health Recovery Board have chosen which may be alerts regarding spikes in overdose deaths in the community, community events, Narcan distribution information, announcements for new services, resources, and/or educational materials provided by the board of public health. New subscribers sign up for the text service and are then opted-in to receive alerts on all topics by default but can adjust their preferences via a link included in the sign-up confirmation text. The applicable Health Department and/or the Mental Health Recovery Board may use the phone number and zip code to understand which areas are utilizing the service and to understand their reach as it relates to distribution of resources.
If you choose to use our LocalHelpNow™ service, you may voluntarily provide us with your zip code. You may also voluntarily complete an online form and provide other personal information in order to be contacted directly by a Provider. This may include a telephone number address or email address. You may be asked for certain medical or insurance information in order to assist the Provider but it is totally up to you what to provide and not provide. The online form leverages a HIPAA compliant email service to securely transmit information submitted by Participants. - Information automatically collected. When you use an Ascend Innovation website including the Ascend Notify™ and/or LocalHelpNow™ service , some information may be automatically collected, such as Internet Protocol (IP) address, operating system, browser type, address of a referring web site, activity on the Site, etc. This information is used by Ascend only in the aggregate. If accessing this site through the internet we may also automatically collect certain information and monitor user activity through the use of "cookies" (including persistent and/or session cookies) A “persistent cookie” is stored on a user’s devise until it expires (persistent cookies are set with expiration dates) or until the user deletes the cookie. Persistent cookies help sites remember your information and settings when you visit them in the future. “Session cookies” are cookies that expire upon the conclusion of your visit to a website. These types of cookies are widely used to facilitate a consumer’s interaction with a website.
